Parent/Guardian Consent and Agreement

 

I, [Parent/Guardian Name], on behalf of the athlete, hereby acknowledge and agree to the following terms and conditions for participation in the OW REGIONAL TRAINING CENTER, Inc. (“OWRTC”) wrestling and COMPETITION camp (“camp”):

 

Assumption of Risks & Participation:

I understand and acknowledge the risks associated with participation in the camp, including but not limited to physical or emotional injury, paralysis, permanent disability, death, and property damage. I accept full responsibility for any injury or damage that may occur and represent that the athlete has adequate insurance coverage or am willing to bear the costs myself. Additionally, I affirm that the athlete has no medical or physical conditions that could compromise their safety during camp activities.

 

Medical Treatment Authorization:

I authorize OWRTC and camp staff to provide medical treatment for the athlete in the event of a medical emergency. I waive any liability for injuries, illness, or skin infections incurred by the athlete while attending camp and acknowledge that there is no trainer/physician present at camp.

 

Athlete Expectation Information:

I agree that the athlete will adhere to the code of conduct outlined by Purler, including participating in all camp activities, following safety guidelines, and respecting camp rules and policies. Failure to comply may result in expulsion from camp without a refund.

 

Photo & Video Authorization:

I consent to the use and reproduction of any photographs or videos taken of the athlete for publicity purposes by OWRTC I understand that Purler owns all electronic images and may use them for promotional materials.

 

Policy on the Responsible Use of Mobile Phones and Digital Devices:

I agree that the athlete will use mobile phones responsibly, following guidelines provided by OWRTC. Phones must not be used to store or transmit inappropriate content, and photography or videography of other athletes or camp staff is prohibited.

 

Quarters:

I acknowledge that damages to camp quarters or facilities may be the responsibility of the athlete, as determined by OWRTC.

 

Release, Waiver, and Indemnity:

I release Purler and the camp from any liability for injuries or damages sustained during camp activities and agree to indemnify and hold them harmless from any losses arising from the athlete’s conduct at camp.

 

By signing below, I affirm that I have read and understand the terms of this agreement and consent to the athlete's participation in the OWRTC wrestling camp.
