The undersigned participant (or parent/guardian), hereinafter referred to as "I" or "my," acknowledges and agrees to the terms and conditions set forth in this Waiver and Release Form. By participating in activities organized by the OWRTC, I understand and willingly accept the risks associated with wrestling activities.

 

Assumption of Risk: I acknowledge that wrestling is a physically demanding and potentially hazardous activity. I am aware that injuries, both minor and severe, can occur during wrestling events, practices, and related activities. I voluntarily assume all risks associated with my (or my child's) participation, including but not limited to falls, contact with other participants, and any conditions of the facilities.

Health and Fitness: I certify that I (or my child) am physically fit and have no medical conditions that would prevent participation in wrestling activities. I agree to inform the coaching staff of any pre-existing medical conditions, allergies, or medications that may affect my (or my child's) participation.

 

Emergency Medical Treatment: In the event of an injury or medical emergency, I authorize the OWRTC and its representatives to obtain necessary medical treatment for myself (or my child). I understand that efforts will be made to contact the emergency contact listed above before such action is taken.

 

Photography and Publicity: I grant permission to the OWRTC to use photographs, video recordings, or any other media taken during team activities for promotional and publicity purposes.

 

Waiver and Release: I, on behalf of myself (or my child), hereby release, discharge, and hold harmless the OWRTC, its officers, coaches, volunteers, and agents from any and all claims, liabilities, demands, or causes of action that may arise out of or relate to my (or my child's) participation in wrestling activities.

 

I have read and understand this Waiver and Release Form, and I voluntarily agree to its terms.
